BACKGROUND: Advance Care Alliance (ACA) is a start-up 501(c)(3) organization that began operations with designation as a Care Coordination Organization/Health Home (CCO) on July 1, 2018. CCO is an emerging care management model for people with intellectual and/or developmental disabilities (I/DD) that combines Medicaid service coordination of home-and community-based services with integrated care management of a person’s medical and behavioral health needs. ACA’s primary mission is to provide a smooth transition to this enhanced service model for people with I/DD and their families. Guided by experienced innovators and advocates of quality care, the organization enrolled more than 25,000 people in its program and draws Care Managers from 100+ member organizations throughout New York City, Long Island, and the lower Hudson Valley. This rapidly-growing organization is increasing staff at the rate of 100 hires per month until July 2019. For more information about ACA’s programs and services please click here.
