Housing Works is seeking dedicated human service professionals to join our expanding community based Case Management Program. For over 25 years the Case Management Programs at Housing Works have provided individuals living with HIV, homelessness, mental health/substance use issues and other chronic illnesses the support they need to improve their health and overall wellbeing. This is an exciting opportunity for career oriented professionals who are committed to social justice and empowerment, and who thrive in a client-centered, outcome-driven work environment.
We currently have the following opportunities available:
- Outreach Worker (HS Diploma/Associates)
- Care Navigator (Bachelor’s Degree)
- Case Manager (Bachelor’s or Master’s Degree)
- Case Management Supervisor (Master’s Degree required)
